Overview
FlexForm Training (“we,” “our,” “us”) provides mobile Personal Training and Stretch Therapy services.
By purchasing a session, booking, or package, you agree to the following Terms & Conditions.
Package Duration & Expiration
All prepaid session packages must be used within 90 days of purchase.
Any unused sessions after 90 days will expire and are non-refundable and non-transferable.
Packages cannot be shared or split between individuals.
This policy is disclosed at the time of purchase and accepted upon payment.
Scheduling, Cancellations & No-Shows
Hour Cancellation Policy
Sessions must be canceled or rescheduled at least 24 hours in advance.
Late cancellations result in the session being marked as used.
No-Show Policy
Failing to attend a scheduled session without notice (“no-show”) will result in the session being forfeited.
Late Arrivals
Sessions end at the scheduled time.
Late arrivals do not receive extended time.
Florida 3-Day Right to Cancel
In accordance with Florida Statute §501.017, you may cancel your package within 3 business days of purchase for a full refund. Cancellation must be submitted in writing (email accepted). Refunds are issued within 30 days.
Death or Permanent Disability
If the client dies or becomes permanently disabled:

The contract may be canceled

A prorated refund will be issued for unused sessions

A doctor’s note (for disability) is required
Medical Pauses & Extensions
Injury or Illness (7+ Days)
If a client is unable to train for 7 days or more due to illness or injury:

A one-time extension of up to 30 days may be granted
-
Requires a doctor’s note confirming inability to train
Non-Medical Reasons
Packages cannot be paused or extended for:

Vacations
-
Scheduling conflicts
-
Work travel
-
Lack of use
Health Disclosure Requirements
Clients must disclose any relevant:
-
Injuries
-
Surgeries
-
Limitations
-
Medical conditions
-
Medications that affect exercise
Failure to disclose may result in increased risk during sessions.
FlexForm Training is not responsible for injuries resulting from undisclosed conditions.
Liability Waiver
By participating in Personal Training or Stretch Therapy sessions, you acknowledge:

Training involves inherent physical risks
-
You voluntarily assume all risks of injury

You release FlexForm Training and its trainers from liability for injuries or complications

This waiver does not apply in cases of gross negligence
Photo & Testimonial Consent (Optional)
FlexForm may use client photos, videos, or testimonials for marketing only if the client provides written consent. Consent is optional and may be revoked at any time.
Mobile Service Policy
FlexForm Training operates as a mobile service in the Fort Lauderdale area.
Clients must provide a clean, safe, and adequate training space.
Refund Policy
Except for the Florida 3-day right to cancel, all sales are final.
No refunds will be issued for:

Unused, expired, or partially used packages

No-shows
-
Late cancellations
-
Inability to complete the package (non-medical reasons)
Acceptance of Terms
By booking or purchasing any FlexForm service, you acknowledge that you have:

Read and understood these Terms & Conditions

Agreed to the 24-hour cancellation policy

Agreed to the 90-day package expiration
-
Understood the risks associated with exercise and stretch therapy

Accepted the liability waiver
